Our urgent care locations now offer the ability to get in line and fill out your registration from the comfort of your home. If you are an urgent care patient, please click the Get In Line button or Online Registration button, choose your location, and complete the online registration. If you are a Wellness Clinic patient, the forms below will allow you to fill out your information prior to arriving for your visit.
